PRIVACY STATEMENT
This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully. All patient/customer information is held in strict confidence. No information related to patients or medication is sold, disseminated or shared to ANY
third party.
Information may be shared between medical professionals. For example, we are allowed to contact your physician to get a prescription for you and to discuss your medications with that physician. We will not, however, discuss medications that you received from other physicians unless there is a drug
interaction. We will not give out any medical information concerning you without your request and your written authorization to do so. We are required by law to maintain the privacy of protected health information.
Your written authorization may be given and can be revoked by you at ANY time. The revocation must also be done in writing. As the patient, you have the right to inspect and amend your private medical records. You also have the right to ask for an accounting of disclosures containing your protected health
information. The Privacy Policy Officer at RxUSA et al, is William Stroud, R.Ph, and Pharmacy Director. All issues regarding privacy information shall be directed to that person. Any complaints should be sent to the Privacy Officer and you also have the right to complain to the Secretary of HHS.
